Lessons for future commissioning
- Commissioners have a critical role in driving quality. Drawing on the enhanced quality standards to be determined by the NHS CB, local commissioners should agree standards above those set by the CQC, with the aim of driving improvement across the system. They should also set out longer term goals with providers by way of developmental standards and focus on improvements in effectiveness.
- It is for commissioners to determine what is to be provided – considering views of clinicians, other providers and procurement expertise. They must also have mechanisms to seek the full involvement of the public and enable their views to be taken into account. For this purpose, commissioners must raise their public profile.
- Information and an understanding of how to act on it are key. The report found that a critical gap in the system of oversight of quality and safety arose from the inability of the commissioners to collect information on provider quality and to understand and make use of the contractual mechanisms that were available to them. On the evidence available, the report found that this was endemic among commissioning organisations. Commissioners should also be responsible for audits, inspections and investigations.
- During the constant change in the NHS, it is essential to put in place systems and processes that can manage the inevitable risks that would occur as the new system establishes itself.
